OSU Medical Residency Program Preceptors

The INTEGRIS Southwest/SWOSU PGY1 Pharmacy Residency Program has 13 active rotation preceptors with over 120 years of combined pharmacy practice experience. The preceptors teach medical students and residents in various clinical areas as well as precept IPPE/APPE SWOSU pharmacy students.

Kristin Montarella, Pharm.D., BCPS

Clinical Specialist, Cardiology
SWOSU Associate Professor of Pharmacy Practice

kristin-montarellaDr. Montarella received her Doctor of Pharmacy degree from Southwestern Oklahoma State University in Weatherford, Oklahoma. She completed an Ambulatory Care residency at Central Arkansas Veterans Healthcare System in Little Rock, Arkansas. In addition to being the residency program director, she is a member of the Residency Advisory Committee and oversees the cardiology rotation in the residency program.  She also serves as the preceptor for the INTEGRIS Southwest Medical Center cardiology rotation for SWOSU pharmacy students.

 

 


Sam Guion, Pharm.D.

Director of Pharmacy

sam-guionDr. Guion received both his B.S. in Pharmacy and Doctor of Pharmacy degree from the University of Oklahoma in Oklahoma City, Oklahoma. He has been affiliated with INTEGRIS Southwest Medical Center (formerly known as South Community Hospital) since 1975 and is currently the director of pharmacy. He oversees inpatient pharmacy services and also serves as the preceptor for the leadership/administration rotation in the residency program.

 

 


Armando Burchett-Zuniga, Pharm.D.

Clinical Specialist, Drug Information

armando-zunigaDr. Zuniga received both his B.S. in Pharmacy and Doctor of Pharmacy degree from the University of Oklahoma in Oklahoma City, Oklahoma. He completed a Drug Information Traineeship at the University of Kansas Medical Center in Kansas City, Kansas. He is a member of the Residency Advisory Committee and serves as the preceptor for the drug information rotation in the residency program.

 

 

 


Blake Bryan, Pharm.D.

Pharmacist, Staff

blakebryanDr. Bryan received his Doctor of Pharmacy degree from Southwestern Oklahoma State University in Weatherford, Oklahoma. He has been affiliated with INTEGRIS Southwest Medical Center since 2007 and is currently a lead staff pharmacist.  He assists with overseeing the staffing rotation in the residency program.

 
 
 
 

Robie Harrington, Pharm.D.

Clinical Specialist, Emergency Medicine

robie-harringtonDr. Harrington received both B.S. in Pharmacy and Doctor of Pharmacy degree from the University of Oklahoma in Oklahoma City, Oklahoma. He has been with INTEGRIS Southwest Medical Center since 2004 as the emergency medicine pharmacist. He oversees the emergency medicine rotation in the residency program and also serves as the preceptor for the INTEGRIS Southwest Medical Center emergency medicine rotation for SWOSU pharmacy students.

 
 

Sara Hawkes, Pharm.D.

Pharmacist, Staff

sarabroxDr. Hawkes received her Doctor of Pharmacy degree from Southwestern Oklahoma State University in Weatherford, Oklahoma.  She has been affiliated with INTEGRIS Southwest Medical Center since 2007 and is currently a lead staff pharmacist.  She assists with overseeing the staffing rotation in the residency program.

 
 
 
 

Ann Nguyen, Pharm.D.

Clinical Specialist, Infectious Diseases

ann-nguyenDr. Nguyen received her Doctor of Pharmacy degree from the University of Oklahoma in Oklahoma City, Oklahoma. She completed a PGY1 Pharmacy Residency and subsequently a PGY2 Infectious Diseases Residency both at the Veterans Affairs Medical Center in Oklahoma City, Oklahoma. She is a member of the Residency Advisory Committee and oversees the infectious diseases rotation in the residency program. In addition, she serves as the preceptor for the INTEGRIS Southwest Medical Center infectious diseases rotation for SWOSU pharmacy students.

 

Ryan Pettway, Pharm.D.

Pharmacist, Information Systems

ryan-pettwayDr. Pettway received his Doctor of Pharmacy degree from the University of Oklahoma in Oklahoma City, Oklahoma. He has been affiliated with INTEGRIS Health since 2008 and is currently the information systems pharmacist at INTEGRIS Southwest Medical Center. He oversees the informatics rotation in the residency program.

 

 

 

Laura Randolph, Pharm.D.

Pharmaceutical Care Pharmacist, Medication Reconciliation

laura-randolphDr. Randolph received her Doctor of Pharmacy degree from Southwestern Oklahoma State University in Weatherford, Oklahoma. She completed a PGY1 Pharmacy Residency at INTEGRIS Southwest Medical Center and is currently a pharmaceutical care pharmacist with a focus on obtaining admission medication histories. She serves as one of the preceptors for the transitions of care rotation in the residency program.

 

 

 

Erica Smith, Pharm.D., CACP

Pharmacist, Anticoagulation Clinic

erica-smithDr. Smith received her Doctor of Pharmacy degree from the University of Oklahoma in Oklahoma City, Oklahoma. She completed a PGY1 Pharmacy Practice Residency at the University of Oklahoma and currently is a pharmacist at the INTEGRIS Southwest Anticoagulation Clinic. She is a member of the Residency Advisory Committee and oversees the ambulatory care rotation in the residency program. She also serves as the preceptor for the INTEGRIS Southwest Medical Center ambulatory care rotation for the University of Oklahoma Health Sciences Center College of Pharmacy students.

 

Cheri Walker, Pharm.D., BCPS

Clinical Specialist, Critical Care
SWOSU Assistant Professor of Pharmacy Practice

She completed a PGY1 Pharmacy Practice Residency and subsequently a PGY2 Critical Care Residency both at Wesley Medical Center in Wichita, Kansas. She is a member of the Residency Advisory Committee and oversees the critical care rotation in the residency program. In addition, she serves as the preceptor for the INTEGRIS Southwest Medical Center critical care rotation for SWOSU pharmacy students.

 
 

Nancy Waugh, Pharm.D.

Pharmacist, Medication Reconciliation

She has been with INTEGRIS Southwest Medical Center since 1988 and is currently a staff pharmacist with a focus on obtaining admission medication histories. She oversees the transitions of care rotation in the residency program and also serves as the preceptor for the INTEGRIS Southwest Medical Center institutional rotation for SWOSU pharmacy students.

 

 

Subi Zachariah, Pharm.D.

Pharmaceutical Care Pharmacist, Oncology

subi-zachariahDr. Zachariah received her Doctor of Pharmacy degree from the University of Florida. She completed a PGY1 Pharmacy Practice Residency at INTEGRIS Baptist Medical Center in Oklahoma City, Oklahoma. She is the oncology pharmaceutical care pharmacist in the outpatient cancer center at INTEGRIS Southwest Medical Center and oversees the elective oncology rotation in the residency program.
